Position Overview
The successful candidate will be responsible for providing oversight of patient-focused care within a pharmacist’s scope of practice for various Kallo Sites and as required, provide direct onsite care or remote care via telehealth interactions.
The Pharmacist - Medication Management will work closely with an inter-professional team as well as a team of pharmacists and pharmacy technicians to optimize medication use and safety through the provision of high quality medication management.
Patient care areas include primary care, emergency care, and public health care (immunizations, infectious and communicable diseases, chronic diseases). Supporting in-hospital pharmacists may also be required, especially during establishment of the hospital pharmacy program. Hospital care areas include acute medical and surgical, obstetrical, general purpose ICU, emergency, isolation, ambulatory and dialysis.
The Pharmacist - Medication Management is expected to have highly developed clinical and communication skills as they work closely with inter-professional teams to provide exemplary care to their patient population. High-level responsibilities include assessing patients and performing comprehensive medication management in conjunction with reviewing laboratory and diagnostic test results and reviewing patient health records using sophisticated clinical information systems.
Accountabilities
In accordance with the laws, regulations and requirements set forth by the Ontario College of Pharmacists or equivalent Canadian Pharmacy College and Kallo Inc, as applicable:
- Check/review patient medications and perform medication reconciliation
- Provide education and counseling to patients regarding medication compliance and potential adverse drug effects, as well as general drug and disease information
- Respond to drug information or current medication requests from patients
- Provide consultation services to other clinical staff
- Compound (as applicable) and dispense medications as prescribed by physicians, nurse practitioners and other health professionals authorized to prescribe
- Provide first or multiple doses and support administration onsite or take-home of selected medications
- Provide follow-up calls or other types of follow-up e.g. mobile SMS texts with patients where indicated/required
- Provide therapeutic drug monitoring
- Promote and implement safe medication practices
- Follow up and complete relevant information on medication incidents
- Create electronic patient medication records and capture other relevant pharmacy data using sophisticated pharmacy information systems and standard templates e.g. eMAR
- Access and review other relevant clinical information in Kallo clinical information systems (CIS) to support clinical decision-making
- Provide direct and/or functional supervision and mentoring of pharmacy technicians and other pharmacy support staff
- Participate in or lead pharmacy or other clinical inter-disciplinary committees as determined by Kallo
- Ensure medications are delivered to the appropriate sites.
- Facilitate and support peer and other student clinical teaching and participate in research activities.

Key Deliverables during First 12 to 24 Months
- Develop Pharmacy strategy and project plan for each of the first 3 years.
- Within the Kallo mobile clinics, set up pharmacy units from the ground up, leveraging retail pharmacy business and operations experience and knowledge to:
- Plan layout and design
- Order and/or approve all start-up inventory
- Develop relationships with relevant vendors e.g. pharmacy suppliers, IT
- Implement standards of practice and operational standards aligned with the requirements of local country professional pharmacy college and/or Ontario or equivalent College of Pharmacy and where applicable, Kallo Inc.
- Develop promotional (advertising and marketing) materials in conjunction with other Kallo team members.
- Manage mobile pharmacy units:
- Manage inventory on an ongoing basis (supplies of medications and other pharmacy items)
- Establish medication management practices; implement workflow processes
- Establish accurate prescription pricing; develop and maintain pricing strategies
- Establish payment methods and sound financial management practices for managing accounts receivable, preparation and reconciliation of 3rd party billings and other pharmacy transactions as applicable
- Interview, hire, train, schedule and perform regular reviews of pharmacy staff
- Develop patient education materials
- Develop training materials to enhance staff usage of the pharmacy information system and related CIS modules as well as other key skill sets
- Motivate staff to take ownership of projects to foster their growth and the growth of the pharmacy’s business
- Perform continual review of internal processes, workflow and operations and measure against business goals and strategies
- Create monthly reports and analysis of same
- Streamline and improve process flows and efficiencies based on the outcome of reviews and analysis of monthly reports; trial innovative processes.
- Facilitate, support set-up of in-hospital pharmacy units, leveraging activities and experience in setting up Kallo mobile clinics
- Develop a high level of competency using Kallo pharmacy information systems and other relevant Kallo clinical information systems to ensure capture/management of relevant clinical data, support high quality pharmacy and medication management and transfer knowledge to other pharmacy staff.
Qualifications and Requirements:
- Possess a Bachelor of Science Degree in Pharmacy or Doctor of Pharmacy degree
- Certificate of competency from the Pharmacy Examination Board of Canada required.
- Active registration with the Ontario College of Pharmacists, Part A or equivalent Canadian College required
- Personal professional liability insurance required and must be maintained on an ongoing basis
- Exemplary work performance and attendance record
- Bilingualism (English/French) preferred
- Additional training or certifications an asset e.g. vaccine administration; compounding
- Hospital pharmacy residency an asset
- Membership in the Canadian Society of Hospital Pharmacists and/or other professional societies an asset.
- 5+ years as a community (retail) pharmacist with excellent working knowledge of retail pharmacy practice in the applicable province including understanding biologics and specialty medications.
- 3+ years + as a community pharmacy manager with significant business and operational experience and success in achieving workflow and operational efficiencies while providing superior patient care.
